For your convenience, a link to NCCS to download the St. George Award Nomination Form. Complete the St. George Award Nomination Form and deliver to CCoS by August 15, 2009. All other warads due no later than September 1, 2009.
Recommended Procedures:
1. Read the letter from Fr. Horkan and Pat Berney. Disregard the submission dates, as the letter was from last year.
2. Review the Award Guidelines on the second page of the letter.
3. Open the nomination form in a word processor. It was prepared in MSWord. For ease, the nomination review committee would prefer you prepare the form using your word processor. Don't worry about spacing, lines or formatting, they know how the form looks. When completed, print four (4) copies and sign at least one and mail or save the completed form as a file and e-mail the file to Yeatsfan@aol.com . Note deadlines on the form.
